Leadership Reporting Forms

The Leadership Report and Lay Delegate Certification Form are distributed annually to collect updated church, clergy and lay leadership contact information. Congregations are asked to submit all three parts of the Congregational Leadership Report within 10 days of their annual meeting as required by diocesan canons and no later than Wednesday, March 1

The information provided through this report is critical to make sure that important diocesan information reaches congregations' current leaders in the upcoming year.

The 2023 reporting components follow below:

  • Part 1: Congregational leadership contact changes are to be reported via an Excel file, which is e-mailed to parochial clergy and administrative contacts on Jan. 25, 2023.
  • Part 2: Church information changes, Licensed Lay Ministers and Safe Church Audit are reported via an online form found here
  • Part 3: Certification of Lay Delegates to Diocesan Convention are reported via an online form found here.
